橫紋肌肌小節的組成除了已知的粗、細肌絲系統外,還包括一種由巨大的蛋白質--肌聯蛋白***TITIN***構成的纖維系統.肌聯蛋白橫跨肌小節從Z線到M線的區域.在心肌中,肌聯蛋白起著分子彈簧的作用,既可以對粗、細肌絲進行精確的調控,產生心肌的被動張力和回覆力,又參與心肌主動張力的調節和維持心肌的緊張度,並在耦聯和協調心肌的舒縮運動中發揮著重要作用.該文主要對肌聯蛋白在心肌舒縮中的功能、訊號傳導機制以及肌聯蛋白與心肌病的關係。肌聯蛋白抗體***Titin-ab***在診斷胸腺瘤重症肌無力***MGT***中的意義。方法採用ELISA法檢測73例不同病理型別MGT和58例不伴胸腺瘤重症肌無力***NTMG***患者血清中Titin-ab和乙醯膽鹼受體抗體***AChR-ab***的水平。結果MGT組患者血清中Titin-ab陽性率為76.7%,明顯高於NTMG組、其他疾病對照組和正常人組***P均<0.01***;Titin-ab診斷MGT的敏感性低於胸腺CT/MR,但特異性高於後者***P<0.05***;MGT患者Titin-ab與AChR-ab有明顯相關性***r=0.496,P<0.01***,而MGH患者則不明顯***r=0.385,P>0.05***。結論Titin-ab是一種診斷MGT***尤其是上皮細胞型胸腺瘤***較為敏感、特異、簡便易行的實驗室引數,它可能在MGT發病過程中起著重要的作用。
